Writing Multiselect Option HTML Code and Collecting the Submitted Data

I am trying to create a form where i need a multiselect option, user can select more than one option from the dropdown and proceed to the next step. I don’t know how to code a multiselect form object and collect the information on successful form submit. Any help here?
